Appointments
If you need to cancel an appointment, we require 24hours notice whenever possible. This enables us to make alternative arrangements, perhaps to see someone in pain. Patients, who habitually break, cancel or arrive late for appointments risk the termination of their registration.
Keep us informed
If you change your address or telephone number, please let us know as soon as possible. This helps to keep our records up to date and our recall system more efficient.
You will be asked to fill in a medical history questionnaire to assist in our diagnosis and treatment. This is confidential.
